Die Wahl des Dialysezugangs hängt von verschiedenen Faktoren ab und ist individuell mit dem Patienten zu besprechen.
Die native Fistel (Dialyseshunt mit eigenen Gefäßen) ist bei jüngeren Patienten die erste Wahl, da weniger Infektionen, Thrombosen usw. zu erwarten sind.
Bei älteren, multimorbiden bzw. sehr adipösen Patienten muss die Entscheidung weitere Faktoren berücksichtigen:
- verzögerte Reifungszeit der Fistel
- Komplikationen und Revisionsoperationen mit weiteren Krankenhausaufenthalten
- Shuntvolumen, das zu einer Einschränkung der Leistungsreserve führen kann
- verschiedene Einflüsse auf die Lebensqualität – Gewichtung individuell bewerten
Bei der Shuntanlage sollte ein Abwägen von Lebenserwartung versus Reifungschancen der Fistel erfolgen:
- d.h. bei älteren und multimorbiden Patienten, mit zum Beispiel einer generalisierten Atherosklerose, kann ggf. statt einer periphere Fistelanlage eine cubitalen Anlage erfolgen
